What happens when you stop semaglutide?

Weight regain after treatment is an important part of the evidence and one reason medical weight management needs a maintenance conversation, not just a starting plan.

Short answer: weight regain can occur after semaglutide is stopped. Follow-up from major weight-management trials has shown that participants regained a meaningful proportion of previously lost weight after treatment withdrawal. Individual outcomes vary.

Why can weight return?

Body-weight regulation is biological as well as behavioural. When a medicine that affects appetite and satiety is removed, the physiological effects of treatment do not necessarily persist unchanged. This is one reason obesity is increasingly managed as a chronic condition rather than a short intervention followed by permanent cure.

What did the STEP evidence show?

Follow-up analyses from the STEP clinical programme examined outcomes after semaglutide withdrawal. Participants regained weight after stopping treatment, although individual trajectories differed. The evidence supports discussing maintenance before treatment ends rather than assuming weight loss will automatically remain unchanged.

Does that mean everyone stays on medication indefinitely?

No single duration is right for every patient. Continued treatment, changing treatment or stopping can depend on effectiveness, side effects, medical circumstances, preferences, access and cost. These are clinical decisions that should be reviewed over time.

What belongs in a maintenance plan?

  • A realistic target for maintaining health improvements
  • Nutrition habits that can continue without medication
  • Regular physical activity, including resistance exercise where appropriate
  • Monitoring for meaningful weight regain
  • A plan for follow-up rather than waiting until substantial regain has occurred
  • Review of the reasons treatment is being changed or stopped

Why Vaya includes a maintain phase

Vaya's model is built around Start, Adjust and Maintain because the long-term question matters. The role of ongoing care is not simply to issue repeat prescriptions; it is to review response and help patients think beyond the initial weight-loss phase.

FAQs

Will all the weight come back?

Not necessarily, and individual outcomes vary. Clinical evidence does show that regain is common after treatment withdrawal.

Should medication be stopped suddenly?

Patients should follow the instructions for their exact medicine and discuss treatment changes with their clinician.

When should maintenance planning begin?

Ideally before treatment stops. Building sustainable habits and defining follow-up while treatment is working is more useful than creating a plan only after regain.
