GLP-1 weight loss treatment in India

A practical guide to GLP-1 medicines, medical assessment, monitoring and the questions patients in India should ask when considering treatment.

In brief: GLP-1 receptor agonists are prescription medicines used for specific medical indications, including some chronic weight-management indications. They are not appropriate for everyone. A registered doctor should assess medical history, current medicines, weight-related risks and treatment goals before deciding whether treatment is appropriate.

What does GLP-1 mean?

GLP-1 stands for glucagon-like peptide-1, a naturally occurring hormone involved in appetite, digestion and blood-glucose regulation. Medicines acting on the GLP-1 pathway can increase feelings of fullness, reduce appetite and alter gastric emptying. Some were developed for type 2 diabetes; particular products have also been studied for chronic weight management.

GLP-1 describes a class or mechanism, not a brand. Semaglutide is one GLP-1 receptor agonist. Ozempic is a brand containing semaglutide. Brand, formulation, strength and approved indication matter.

What do clinical studies show?

Clinical trials of semaglutide 2.4 mg for chronic weight management have reported substantial average weight reduction when combined with lifestyle intervention. The STEP 1 trial published in the New England Journal of Medicine is a frequently cited example. Trial averages are not predictions for an individual; outcomes vary with tolerance, adherence, nutrition, activity, other medicines and underlying health.

Who may be considered?

Weight-management medicines are considered in the context of BMI, weight-related conditions, previous weight-management efforts and overall clinical risk. Regulatory indications differ by product. CDSCO committee material concerning chronic weight management has referenced adults with BMI of 30 kg/m² or greater, or 27 kg/m² or greater with at least one weight-related comorbid condition.

A medical assessment may cover diabetes, blood pressure, cholesterol, sleep apnoea, cardiovascular history, gastrointestinal symptoms, kidney health, pregnancy plans, mental health, family history and other medicines. How does a doctor-led pathway work?

The process begins with a clinical conversation. A doctor considers whether medical weight management is appropriate and discusses options, expected benefits, limitations and risks. Why does dose management matter?

Many GLP-1 medicines use a staged dosing schedule. The aim is not simply to reach a higher dose quickly. Follow-up allows tolerability, side effects and response to be reviewed. Patients should follow the instructions supplied with their medicine and seek clinical advice about dose changes, missed doses or restarting treatment.

Side effects and safety

Gastrointestinal effects such as nausea, vomiting, diarrhoea, constipation and abdominal discomfort are commonly reported. Warnings differ by product. Severe or persistent symptoms require medical attention. Patients should disclose all medicines and supplements and read the product information supplied with the medicine.
